Before your first visit to our office, take a minute to print and fill out your new patient forms. This will save time and help us keep the most up-to-date information on file. Below, you can download both the Health History Form and the Patient Information Form. For any questions or assistance, please call our office.
Financing
At Advance Dental, we offer many financial options to help you afford your dental needs. From offering flexible payment options to working with your insurance, we will do everything we can to help you make it possible to afford your dental work.
Often, we find that families put off dental care due to financial stress. We don’t want your smile to suffer because of a lack of or the high cost of dental care. If you do not have dental insurance, Sunbit and CareCredit may be a great option to help you to get the dental treatment you need.
We are happy to offer Sunbit and CareCredit for our patients to allow them to pay off their dental care over time. Sunbit and CareCredit can be used on most treatments that improve oral health, such as dental implants, root canals, teeth whitening, veneers, crowns and more.
